Key Responsibilities

The physical therapist will provide one-on-one care to patients in their homes by developing and implementing personalized therapy plans. They will collaborate with a multidisciplinary team, including physicians and nurses, to monitor patient progress and ensure high-quality clinical outcomes.

Requirements

Candidates must hold a bachelor's degree in physical therapy from an APTA-accredited program and possess a current Connecticut physical therapist license. A minimum of one year of experience or completion of a preceptorship is required, along with a valid driver's license.
