Key Responsibilities

Patient Care: Conduct initial and ongoing evaluations, including OASIS assessments.

Develop and update care plans with the physician and care team.

Identify barriers, set goals, provide treatment, education, and equipment.

Coordinate services and plan discharge.

Salary: $75 - $85/visit

Work Schedule

Per Diem position.

Day shift only - no nights.

Flexible scheduling available.

Requirements

Candidates must possess an active Colorado Occupational Therapist license and be a graduate of an approved occupational therapy curriculum. Experience in Home Health is preferred, but not required, and a valid Driver's License with a clean Motor Vehicle Record is mandatory.
