Requirements

Unrestricted Illinois license and DEA prior to start
Board Certified
2+ years of provider work experience is strongly preferred.
Master’s degree in Nursing is required.
Bachelor’s Degree in Nursing PLUS 5 years relevant experience may be considered in lieu of degree.
Doctoral degree in Nursing is preferred.
