Key Responsibilities

Provide skilled nursing and head-to-toe assessments to the adult and geriatric population.

This includes the oversight of medication administration and wound care treatment as prescribed in the physician's plan of care.

Provide visiting home health services to an assigned caseload including admissions, re-certification and discharges.

Develop the Plan of Care in coordination with all health care professionals assigned to the case.
