Key Responsibilities

Provide home

based social services to patients dealing with end

life issues

Work as part of a hospice team, which will include an RN, LPN, CNA, Spiritual Counselor and Hospice Aides focused on the patient’s plan of care and individual goals

Benefits & Compensation

Competitive Pay – $60,000 annually

Flexible Hours – typical work hours are Monday

Friday 8:00 AM

5:00 PM with flexibility as needed

Work Schedule

Full-time position.

No weekends required.
