Key Responsibilities

Administer physical therapy treatment plans to patients in their homes to help them regain strength, mobility, and independence. Document client records and coordinate with the physical therapist regarding patient progress and equipment needs.

Requirements

Requires a current state license to practice physical therapy, a valid driver's license, and current CPR certification. Candidates must have experience collaborating with healthcare teams and possess knowledge of geriatric care.
