The Social Worker is accountable for providing medical social casework to diverse patients, focusing on meeting psychosocial and continuum of care needs for the patient and family to ensure optimum functioning. Duties include direct care services, consultation, education, policy planning, performance improvement tasks, advocacy, and community liaison.
Candidates must possess a Bachelor's in Social Work and hold an active Social Worker license in the state of practice upon hire. Preferred qualifications include 1-3 years of healthcare social work experience, and the individual must be able to perform essential job functions satisfactorily.
