Key Responsibilities

Collect clinical data for patients, caregivers, and nurses and assess data to monitor efficacy and safety of a prescribed specialty therapies
Collaborate with prescribers to implement or modify existing treatment plans
Embraces Soleo’s Quality Assurance and Performance Improvement initiatives, and plays an active role in supporting these programs

Requirements

Obtains additional state pharmacy licenses as requested
Must be licensed in the state of Kanas as a Pharmacist
Prior experience in home infusion and/or specialty pharmacy preferred
Doctor of Pharmacy (Pharm D) or R. Ph with relevant clinical experience
Additional specialty certification or training may be required
Prior experience with CPR+ pharmacy computer system preferred

Work Schedule

No Weekends or Holidays
Full Time, 40 hours per week, Monday to Friday, 8:30am - 5pm
On-Call 1 week every 3 - 4 months
