Key Responsibilities
- & RESPONSIBILITIES Maintains confidentiality of all patient records.
- Checks in with clients about test results and prognoses.
- Advises patients regarding prescriptions and treatment plans.
- Follows all federal, state, and nursing guidelines, regulations, and standards.
Requirements
- Based Care experience, including experience with quality measures, risk
- based/panel management, care coordination, outcomes
- based care delivery, and documentation practices that support accurate clinical capture and continuity of care. Preferred Clinical Experience (Renal/Nephrology)
- Renal and/or nephrology experience is highly preferred, including familiarity with CKD management, hypertension, diabetes

- Full-Time position.
