- Provides strength
- based, recovery oriented direct therapeutic interventions to clients, in person, in the community and/or by telehealth, to clients, using evidenced
- based practices.
- Performs patient and family
Requirements
- Serves on the Multidisciplinary Team and participate in regular Quality Improvement and Utilization Review activities to drive process improvements and enhance quality of care.
- Coordinates treatment and care with other providers, including for example, primary care, medical specialists, or other programs like day treatment.
- Works as liaison between Vinfen programs, state agencies, funding sources, and community referral sources.
- Participates in educational trainings, seminars, grand and clinical rounds.
