Key Responsibilities

• Must possess verbal and written communication skills to effectively communicate with patients and family members in a caring and empathetic manner.

• will be a core member of the hospice Interdisciplinary team who provides supportive services to help meet the hospice patient’s and family’s social, emotional and economic needs in relation to the impact of a terminal illness.

• will coordinate safe patient transition from hospital to home or another facility, arrange services, collaborate with doctors, nurses and insurers, lead hospital interdisciplinary meetings, facilitate the implementation and referral to community resources, etc.

Requirements

• Graduate of an approved Bachelor’s Degree program in social work.

• Experience, preferred.

• However, new graduates welcome.

Benefits & Compensation

• Salary: $24.64 - $30.93/hr
