Key Responsibilities

• will partner with the interdisciplinary team (Behavioral Health, PSAR, pharmacists, PCP's, RN Care Managers) to help patients access appropriate care.

• must possess current knowledge of the bio psychosocial aspects of illness, disability and death and be able to apply this knowledge in a professional and understanding manner in direct social work services to patients and their families and in collaboration with all health care professional staff.

Requirements

• Master's Degree Social Work

• Upon hire: Washington Clinical Independent Social Worker Associate License Or

• Upon hire: Washington Clinical Independent Social Worker License

• Experience in Case management.

Benefits & Compensation

• Salary: $80K - $124K

• 401(k) with employer match.

Work Schedule

• Full-Time position.
