  • High client satisfaction with rehabilitation and transition supports
  • Increased proportion of clients safely transitioning to their residence or appropriate alternate housing Program & Evaluation Outcomes
  • Demonstrated feasibility of the OT

Requirements

  • Evidence to inform scalability of virtual clinical service coordination
  • Conducts comprehensive occupational therapy assessments across diverse settings, including hospitals, seniors’ residences, long
  • term care, and client homes.
  • Develops individualized, measurable, and time
