Key Responsibilities

Conduct face

face patient visits to evaluate continued hospice eligibility in line with new regulations.

Collaborate with physicians and the interdisciplinary team to share clinical findings and adjust care plans.

Perform bedside evaluations, creating comprehensive care plans tailored to the patient’s needs.
