About the Role
Join Monogram Health as a Lead Palliative Care Physician – In-Home, Sign-On Bonus in Nashville, Tennessee. POSITION: LEAD PALLIATIVE CARE PHYSICIAN Monogram Health is seeking a full-time physician specializing in palliative care to join our team amid continued nationwide growth. This is an in‑market, field‑based position requiring residence within one of our approved territories.
Key Responsibilities
- Collaborate with MPE and the care team to proactively identify patients who qualify for palliative care services and build a patient panel
- Developing and presenting metrics to executive leadership, as requested
- Responsible for the outcomes for patient panel, driving both predetermined clinical and financial Key Performance Indicators (KPIs) through interventions outlined below

- Must be willing and able to obtain hospital privileges at required facilities.
- Desire to practice in a forward
