About the Role

Key Responsibilities

• Perform comprehensive health assessments and physical examinations during in-home visits and telehealth appointments.

• Evaluate, diagnose, and treat acute and chronic conditions with an emphasis on preventive care.

• Develop and implement individualized treatment plans tailored to each patient’s needs.

• Educate patients and families on health management, wellness, and disease prevention.

Requirements

• based care experience strongly preferred. Experience caring for patients with disabilities is highly desirable. Comfortable delivering care in both in

• home and telehealth settings. Excellent communication skills and a collaborative, team
