About the Role

Join Jefferson Health Plans as a Social Worker- NavSTAR in United States.

Key Responsibilities

will compare proactive Patient Navigation services to usual care for hospital patients at Thomas Jefferson University Hospitals with opioid use disorder, a group at high risk of repeat hospitalization and negative outcomes.

will work with discharged hospital patients in the community to resolve barriers to engaging in recommended follow-up medical and addiction care.

will also provide guidance and support to other members of the NavSTAR Patient Navigation team.
