Key Responsibilities

AND FUNCTIONS: Participant Care Performs participant assessments to facilitate plan of care, treatment, and discharge planning in collaboration with the interdisciplinary team.

Reviews and evaluates drug information to identify potential medication therapy problems and recommends appropriate solutions to problems.

Communicates participant changes to team members and documents in plan of care.

Requirements

Candidates must be a Registered Pharmacist with a Controlled Substance license in the State of Michigan, holding a Doctor of Pharmacy degree or equivalent experience demonstrating advanced clinical knowledge. A PGY1 Pharmacy Practice Residency focused on ambulatory care or geriatrics, along with Board Certification in Geriatric or Ambulatory Pharmacy, is preferred.
