Requirements

Candidates must be a Registered Pharmacist with a Controlled Substance license in the State of Michigan, holding a Doctor of Pharmacy degree or equivalent experience demonstrating advanced clinical knowledge. A PGY1 Pharmacy Practice Residency focused on ambulatory care or geriatrics, along with Board Certification in Geriatric or Ambulatory Pharmacy, is preferred.
