Join Pennant Group as a Home Health Occupational Therapist – Flexible Schedule in United States. Join Puget Sound Hospice as a Occupational Therapist! Position: Full-time or part-time Home Health Occupational Therapist (OT) Location: Thurston County.
Key Responsibilities
- Provide comprehensive patient care through functional assessments, identification of performance deficits, and development of client‑centered treatment plans in home health settings.
- Develop and implement individualized OT care plans focused on improving ADLs, IADLs, safety, participation, and environmental adaptations.
- Educate patients and families on therapeutic techniques, compensatory strategies, energy conservation, home exercise/engagement programs, and safe use of adaptive equipment.
- Collaborate with interdisciplinary teams to ensure continuity of care, effective communication, and safe discharge planning.
