About the Role

Join P3 Health Partners as a Nurse Practitioner – Home Visits, On-Call in United States.

Key Responsibilities

Conduct comprehensive in

Perform evidence

based practice screenings including point of care testing (as appropriate).

Identify diagnoses to be used in care management and active medical management in the furtherance of treatment.

Requirements

Medical/Master’s Degree and Certification

Maintain a controlled substance license along with an unrestricted State DEA license

Minimum of 3 years of nursing experience

Valid State Driver’s License with a clean DMV record
