About the Role

Join Amana Living as a Social Worker – Full-Time, M-F Schedule in United States. The Role We currently have an exciting opportunity for a highly motivated, person orientated individual to join our organisation as a Social Worker. This is full-time, spanning across two of our facilities: Lefroy Care Centre (BullCreek) & Thomas Scott Hostel(Bull Creek), Monday - Friday.

Key Responsibilities

Undertaking social work assessments, including psychosocial assessments for all assigned clients in consultation with other team members

Liaising with clinical staff/ pharmacy and informing them of residents’ planned discharge dates for transport arrangements

Case management may include supporting clients with State Administrative Tribunal (SAT) and guardianship processes.

Planning and undertaking social work interventions with clients, in conjunction with their families and the multidisciplinary team
