The Nurse Practitioner conducts comprehensive in-home and telehealth health risk assessments to identify chronic conditions and physical needs. They are responsible for documenting patient visits, managing referrals, and communicating with the care team to ensure proper diagnosis and treatment.
Candidates must hold an active, unrestricted Nurse Practitioner license and board certification from ANCC, AANP, or NCCPA. A valid driver's license, active BLS certification, and proficiency in EHR systems are also required.
