Key Responsibilities
- Triages referrals from providers and ensures on-going liaison occurs with stakeholders as applicable to assigned area(s).
- Performs as part of a multi-disciplinary treatment team, provides consultation, and education services as applicable to assigned area(s).
- Coordinates patient transitions into services and discharges from treatment as applicable to assigned area(s).
