• Qualifications: Candidate must have a Masters in Social Work (MSW) At least 1 year experience providing social work services in an acute care setting required.

• Strong leadership ability and excellent interpersonal skills, as well as, solid oral and written communication skills.

• Manages the placement process into inpatient facilities.

• Reviews financial resources for long term placement and provides education to the patient/family.

• Also develops plans for transfer to SNF, acute rehabilitation facilities, assisted living, inpatient substance abuse centers and inpatient psychiatric hospitals.

• Facilitates patient/family and provider meetings as necessary to develop and plan strategies related to psychosocial and economic issues, particularly those related to care progression and transition.

Work Schedule

• Full-time position.

• Day shift only - no nights.
