About the Role
Join UnityPoint Health as a Pharmacist- Downtown in United States.
Key Responsibilities
- Verifies all orders generated on shift, keeps work flow up to date and current, and cleans up old orders as soon as possible.
- Completes verbal hand off communication to next shift, to ensure continuity of quality patient care and follow through in interventions.
- Screens and affirms appropriateness of orders including allergy check, formulary compliance, pregnancy & lactation status, dosing, duplicate orders etc.
- Documents adverse drugs reactions & medication errors in the RL system as they occur throughout the medical center.
Requirements
- related reasons required. Board Certified Oncology Pharmacist (BCOP) or Board
- Certified preferred. Pharmacotherapy Specialist (BCPS) or Board
- Certified Ambulatory Care Pharmacist (BCAPS) preferred.
