About the Role
Join ATA Services Inc as a Social Worker IV – Forensic Psychiatry in Kaneohe, Hawaii, United States. ATA Services, Inc., is looking for a full-time Social Worker IV for our client Hawaii State Hospital.
Key Responsibilities
- Performs evaluation of admission materials and obtains data essential for treatment planning and documents same in a full psychosocial admission assessment according to current department guidelines.
- Collaborates with treatment team members to determine whether further diagnostic studies or information are needed.
- Participates in developing individual Master Treatment Plans with treatment team members and take the lead in development of a transition and discharge plan.
- Coordinates the completion of all applications for discharge including housing, financial, work, and psychosocial outpatient programs.
Requirements
- * In addition to meeting the requirements specified for level III, applicants must have had a minimum of one additional year of professional social work experience equivalent to level III.
- Such experience must have demonstrated ability to perform the most difficult and complex assignments.
