About the Role

Join ATA Services Inc as a Social Worker IV – Forensic Psychiatry in Kaneohe, Hawaii, United States. ATA Services, Inc., is looking for a full-time Social Worker IV for our client Hawaii State Hospital.

Key Responsibilities

Performs evaluation of admission materials and obtains data essential for treatment planning and documents same in a full psychosocial admission assessment according to current department guidelines.

Collaborates with treatment team members to determine whether further diagnostic studies or information are needed.

Participates in developing individual Master Treatment Plans with treatment team members and take the lead in development of a transition and discharge plan.

Coordinates the completion of all applications for discharge including housing, financial, work, and psychosocial outpatient programs.

Requirements

* In addition to meeting the requirements specified for level III, applicants must have had a minimum of one additional year of professional social work experience equivalent to level III.

Such experience must have demonstrated ability to perform the most difficult and complex assignments.

Salary: $37/hr
