Key Responsibilities

• Establishing a care plan in accordance with the physician’s plan of treatment.
• Establishing a care plan in accordance with the physician’s plan of treatment

Requirements

• Providing crisis intervention as required.
• Completing and submitting all required documentation in an accurate and timely manner.
• Master’s degree from an accredited school approved by the Council on Social Work Education. In Indiana, must also be certified under IC25
• Currently licensed to practice in the state serviced. (LICSW/ LISW) required
• Current CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements
• Master’s degree from an accredited school approved by the Council on Social Work Education
