Key Responsibilities
- Regimen review, ensuring safe medication administration, monitoring for desired therapeutic outcomes, identifying and avoiding medication errors and adverse drug events, etc.
- Provides pharmacological information by answering questions and requests of health care professionals
- Works collaboratively with Management staff in the medication management of patients transitioning from inpatient to outpatient or next level of care
- Counseling patients on drug therapies
Requirements
- Graduate of school of pharmacy with a Doctor of Pharmacy (Pharm. D.) degree.
- Current licensure as a pharmacist in the State of California
- Ability to exercise independent judgment and prioritize a diverse workload.
- Outstanding organizational and communication skills.
