About the Role
Join Stamford Health as a Pharmacist – Full-Time Hospital in United States. The Pharmacist reports to the Director of Pharmacy, this position is responsible for providing quality pharmaceutical care for the patients of The Stamford Hospital with an emphasis on accuracy and patient safety. Schedule: 37.
Key Responsibilities
- Accurately verifies, prepares and dispenses medication orders per physician request according to established policies/procedures/protocols and based on pharmaceutical knowledge 2.
- Provides timely and accurate drug information to healthcare professionals and lay persons when required 3.
- Participates in medication counseling sessions for patients by reviewing side effects, indications and directions for use 4.
- Demonstrates knowledge and skills necessary to provide quality pharmaceutical care to all patients.
Requirements

- Ensures safe and appropriate cost effective medication therapies for patients according to established policies, procedures and protocols.
- Assists with pharmacokinetic consult services and drug dosing per organizational protocol 13.
- Ensures that therapeutic substitution policies are followed based on individual patient need.
