About the Role

Join HIA Health as a Hospice Social Worker – Fargo, ND, Day Shift in United States.

Key Responsibilities

  • Conducts hospice referrals and accurately completes admission paperwork/consents.
  • Conducts hospice comprehensive assessments at the time of admission.
  • Assess the patients and family’s social emotions and financial needs.
  • Assesses the patient’s/family’s psychosocial needs on an ongoing basis while hospice care is being provided through comprehensive assessment updates.

Requirements

  • Refers PCG and family to bereavement in an appropriate manner.
  • Completes and locks documentation on the same day as the contact/communication occurs as per the Medical Records Documentation policy.
