About the Role

Join Yale New Haven Health as a Per Diem Pharmacist in United States.

Key Responsibilities

  • Resolves drug distribution problems assuring accurate and timely dose delivery to all patients 1.
  • 1 Responds to telephone requests for medication in appropriate fashion accommodating callers request or routing call to appropriate person.
  • Interprets Clinical and Pharmacokinetic Lab Data in order to formulate medication monitoring plans.
  • 1 Demonstrates knowledge of basic pharmacokinetic principles.

Requirements

  • EDUCATION Graduation from an accredited College of Pharmacy with a B.
  • Completion of a residency in hospital pharmacy from a program accredited by the American Society of Hospital Pharmacists is desirable but not required.
  • EXPERIENCE Two years experience in an acute care hospital pharmacy is preferred for this position.
