About the Role

Join Yale New Haven Health as a Per Diem Pharmacist in United States.

Key Responsibilities

Resolves drug distribution problems assuring accurate and timely dose delivery to all patients 1.

1 Responds to telephone requests for medication in appropriate fashion accommodating callers request or routing call to appropriate person.

Interprets Clinical and Pharmacokinetic Lab Data in order to formulate medication monitoring plans.

1 Demonstrates knowledge of basic pharmacokinetic principles.

Requirements

EDUCATION Graduation from an accredited College of Pharmacy with a B.

Completion of a residency in hospital pharmacy from a program accredited by the American Society of Hospital Pharmacists is desirable but not required.

EXPERIENCE Two years experience in an acute care hospital pharmacy is preferred for this position.

Work Schedule

Per Diem position.
