Key Responsibilities

Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

Provide service within the scope of practice as defined by the state laws governing the practice of Occupati.

Conduct patient assessments evaluating the level of function by applying diagnostic and prognostic functiona.

provide healthcare services through home health and hospice agencies and senior living communities located t.

Requirements

S: Current PT license in the state where services are rendered.

Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
