Key Responsibilities

• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

• Provide social work services in accordance with the plan of care.

• implement appropriate interventions to reduce risks.

• provide healthcare services through home health, hospice, and home care agencies and senior living communiti.

Requirements

• S: Master's degree (MSW) or (BSW) from a school of Social Work accredited by the Council on Social Work Education.

• Must have a current and unencumbered Social Worker License in the state where services are rendered.

• Must have one (1) year of social work experience in a health care setting.

• Current CPR certification, driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
