About the Role
Join Humana as a Field Care Manager – Remote, No Weekends in United States.
Key Responsibilities
- Manage a caseload and complete comprehensive assessments with members in their home or community‑based settings, as well as telephonically.
- Provide clinical support and guidance, collaborating with Interdisciplinary Care Team (ICT) participants to support members with medical complexity.
- Develop and coordinate individualized care plans, ensuring members receive appropriate services to effectively manage their health needs.
- Identify and address barriers to care while advocating for optimal member outcomes.
