About the Role
Join Larkin Community Hospital as a Pharmacist – Per-Diem Flexible in United States. Skills/Experience Required: Job Summary: Responsible for providing comprehensive, patient-centered, clinical pharmacy services for assigned areas. The Clinical Pharmacist proactively influences medication use and makes recommendations concerning the patient’s medication profile.
Key Responsibilities
- will assess medication orders for the appropriateness of therapy, taking into account patient-specific factors, and evaluate medication regimens to ensure positive outcomes, including relevant laboratory results, tests, and/or procedures.
- will initiate and participate in the collaborative management of patients with other healthcare team members, including medication reconciliation, adherence to core measures, anticoagulation management, targeted drug programs, medication usage guidelines, patient counseling, discharge counseling, and performance improvement initiatives.
- will participate in continuous readiness efforts to maintain Joint Commission Patient Safety Goals and Medication Management Standards.
- will serve as a primary resource for drug information to patients, medical staff, nursing staff, and personnel from ancillary departments.
Requirements
- Doctorate of Pharmacy degree or Bachelor of Science degree or equivalent.
- Graduated from an accredited School of Pharmacy.
- Current (ACTIVE) Pharmacist Licensure in the State of Florida. Work Schedule: Flexible
