About the Role
Join InnovAge as a Nurse Practitioner - Triage Team in United States. This role ensures timely intervention and serves as a clinical leader for triage operations.
Key Responsibilities
- Serve as the primary clinical resource for urgent participant needs within the PACE center.
- Perform rapid assessments, physical exams, and order/interpret diagnostic tests for triage cases.
- Formulate treatment plans, prescribe medications, and provide immediate interventions as needed.
- Manage participant and staff inquiries, prioritizing care and routing appropriately.
