Key Responsibilities
- Conduct a comprehensive assessment for the client relevant to biopsychosocial history, diagnosis and ability.
- Develop and evaluate treatment plans in collaboration with the clinical team, community supports and familie.
- Provide individual counselling in support of patient’s specific needs and goals * Ensure continuity of care.
Requirements
- Master’s Degree in Social Work
- Current registration with the Ontario College of Social Workers
- Training and educated in CBT and/or DBT modalities, and Mindfulness
- Experience working with individuals with complex mental health and addiction issues, and trauma
