About the Role

Key Responsibilities

  • Meet with clients and families to assess needs related to trauma and care coordination.
  • Collaborate with clients and team members to develop individualized care plans.
  • Manage a caseload of clients, identifying therapeutic, advocacy, and care needs while engaging team support.
  • Assist with applications for orders of protection and provide court support for clients.

Requirements

  • Master’s degree in Social Work plus a minimum of 2 years’ experience in a mental health setting.
  • Licensed Clinical Social Worker (LCSW), LMHC, or LMFT required.

  • Ability to work collaboratively with multidisciplinary teams.
