Key Responsibilities

Assess resident psychosocial, emotional, and medically related social needs.

Provide direct social services or connect residents with appropriate community resources.

Assist residents with financial assistance applications and outside agency referrals.

Maintain accurate, confidential social service documentation and progress notes.

Requirements

Bachelor’s degree in Social Work, Gerontology, or a related field.

Current Social Worker license in the state of CT.

Minimum of 2 years’ experience in senior services or a related field.

Strong communication, documentation, and interpersonal skills.

Salary: $74,880 - $79,040/year
