About the Role

Join JPS Health Network as a Family Medicine Physician – Part-Time, Outpatient Clinic in United States. Acclaim Physician Group, Inc. (Acclaim) is a multi specialty medical group practice comprised of over 500 physicians, nurse practitioners, and physician assistants that partner with JPS Health Network (JPS).

Key Responsibilities

and Essential Job Functions: · Coordinates the multidisciplinary care delivered to their panel of patients, to optimize patient care.

· Reviews patient medical records to diagnose, treat, and provide care management and education to patients and families.

· Makes appropriate patient referrals.

· Monitors national health guidelines and those established by JPS Health Network and Acclaim Multispecialty Group for the management of patient care.
