Join Helping Hand Nurse, LLC as a Occupational Therapist- Home Care (Trevose) in United States. Helping Hand Nurse, LLC is a home health care agency seeking Physical Therapists for our home health patients. We are actively hiring in multiple locations across Lebanon County.

Key Responsibilities

will provide physical therapy services to patients according to a written physician's plan of care which will, in part, include: Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care, as well as evaluating the home environment.

will be responsible for visiting patients in their homes and providing skilled home health care clinical visits, providing care for individuals based on personalized Plan of Care.

Requirements

/Requirements:(preferred unless required by law) Current unrestricted PT license Minimum one (1) year of home care experience preferred.

Three (3) to five (5) years of clinical experience and preferred previous supervisory experience.
