Key Responsibilities

• Function as the primary liaison with DMH, Behavioral Health Community Partners (BH CP), One Care and/or DMH Case Managers for care coordination integration.

• Monitor staff performance and productivity. Evaluate staff performance, including offering consistent and direct performance feedback by completing competency
