About the Role
Join Ohio s Hospice Inc as a Medical Social Worker in United States.
Key Responsibilities
- Assesses, evaluates and documents the patient and family needs for biopsychosocial, safety, financial, bereavement, and anticipatory grief.
- Develops and implements individualized plans of care including creating measurable patient
- centered goals in collaboration with the patient and family.
- Participates as a member of the hospice team in regularly scheduled care conferences, hospice admissions, and Interdisciplinary Team (IDT) meetings, to assist with the coordination of services.
Requirements
- Licensed Social Worker with the state of Ohio LISW preferred or LSW.
- Master's Degree in Social Work or related degree as required for LSW licensure.
- One year social worker experience in a health care setting preferred.
- Computer skills sufficient to properly document services and care.
