Key Responsibilities

• Conduct home visits to clients in designated geographic areas.
• Perform evaluations to assess speech, language, communication, or swallowing difficulties.
• Develop and revise treatment plans based on evaluations and physician orders.
• Educate patients and families on rehabilitative care.
• Document care management accurately using electronic tablets.

Requirements

• Current Colorado license as a Speech Language Pathologist.
• Certificate of clinical competence from the American Speech and Hearing Association or equivalent education and experience.
• Solid computer skills; EMR experience preferred.
