Requirements

Candidates must hold a Master's degree in nursing, a current Arizona Nurse Practitioner license, and board certification as a Family Nurse Practitioner. Additionally, 1–3 years of relevant experience, current DEA/ANCC certifications, and a valid Arizona Fingerprint Clearance card are required.
