Requirements

Candidates must possess a PharmD or B.S. in Pharmacy, with a residency being preferred, and must hold an active Indiana Pharmacist License or Pharmacy Intern License. A minimum of two years of hospital pharmacy experience or completion of a residency is required for this role.
