About the Role

Join CareConnectMD Inc as a Nurse Practitioner - Palliative in United States. Key Duties and Responsibilities * Assists with the provision of palliative care in accordance with local, state, and federal regulations. * Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs.

Implements interventions to support the patient with chronic conditions to decrease recurring hospitalizations.

Requirements

Must work well independently and within a team.

Strong computer skills, including Word, Excel, and PowerPoint.

At least 2 years NP experience with hospice and/or palliative care experience (either NP or RN).

Current Advanced Practice Registered Nurse (APRN) Licensure.
