Key Responsibilities

Provide consultative and direct patient care in palliative and hospice settings (home, skilled nursing facility, inpatient)
Collaborate with physicians, advanced practice providers (APPs), nursing, social work, spiritual care and case managers.
Participate in quality improvement initiatives, program development, and education of medical staff.
Professional development and continuing education opportunities

Requirements

Board certified or board eligible in Internal Medicine or Family Medicine
Fellowship trained and/or Board Certification in Hospice & Palliative Medicine preferred
Active or eligible for Connecticut medical license
