Key Responsibilities
- Provide consultative and direct patient care in palliative and hospice settings (home, skilled nursing facility, inpatient)
- Collaborate with physicians, advanced practice providers (APPs), nursing, social work, spiritual care and case managers.
- Participate in quality improvement initiatives, program development, and education of medical staff.
- Professional development and continuing education opportunities

Requirements
- Board certified or board eligible in Internal Medicine or Family Medicine
- Fellowship trained and/or Board Certification in Hospice & Palliative Medicine preferred
- Active or eligible for Connecticut medical license
