Clinical Pharmacist, Multidisciplinary Inbasket Support Team (MIST) - Work From Home

Key Responsibilities

The Clinical Pharmacist will perform remote refill assessments and adjust medication therapy for chronic disease states under a collaborative practice agreement. They are responsible for providing clinical assessments of patient home readings and supporting providers with medication-related inquiries.

Licensure & Certifications

Candidates must hold a PharmD degree from an accredited pharmacy school and possess at least 3 years of recent relevant experience. Current pharmacist registration is required, with board certification and Advanced Practice Pharmacist licensure preferred.
