Essential Addendum (essential for specific dept) OFF CAMPUS: Performs all the principal duties of a Pharmacist-In-Charge.

Establish policies and procedure for the department.

Reviews and revises policies and procedures at least once a year.

Participates in coordination of principal department operations, including committee functions.

Requirements

License /Certification Required: Current California Registered Pharmacist license Preferred: Pharmacy Practice Residency Certificate Education Preferred: Pharm.

Prefer completion of a residency program in home infusion pharmacy.
